Allianz Care: cover benefit by benefit
IPMIBenefits reads Allianz Care's international plans benefit by benefit: what sits in the three core levels, what the optional out-patient and dental modules add, and why the choice of underwriting basis matters as much as any benefit limit.
- Out-patient and dental are optional modules bought at the same level as the core plan you choose.
- Moratorium underwriting carries a 24-month waiting period on pre-existing conditions; full medical underwriting gives certainty from day one.
- Skipping required pre-authorisation means an eligible claim is paid at 80% for inpatient and 50% for other benefits.
- Emergency admissions must be reported within 48 hours and invoices submitted within 60 days of discharge.
- Leaving the USA out of the area of cover is the biggest single saving on the plan.
Plans and structure
Allianz Care is the international private medical insurance (IPMI) arm of Allianz. Its individual plans follow a core-plus-modules pattern: you choose one of three core levels, then decide whether to add out-patient cover and dental cover. The published rule that shapes everything else is that the modules must sit at the same level as the core you pick, so a Care Enhanced core takes Care Enhanced modules.
| Core plan | Position in the range |
|---|---|
| Care Base | Entry-level core cover |
| Care Enhanced | Mid-tier, with broader limits and benefits |
| Care Signature | Allianz's most comprehensive level |
Maximum plan limit: Care Base US$1,350,000 (£830,000), Care Enhanced US$2,700,000 (£1,660,000), Care Signature US$5,000,000 (£3,100,000). Areas of cover: Worldwide, Worldwide excluding USA, Africa only and Europe (Allianz Care Table of Benefits, Care, valid from 1 November 2025, pp.6 and 20, read 19 October 2026). Leaving out the USA, if you do not need treatment there, is the biggest single saving on the plan.
How to read this structure: the core level sets the depth of hospital cover, and each module you add brings a second decision about level. The Table of Benefits for your core level is the document to compare against the limits and waiting periods other insurers publish.
What the core and the modules cover
- Inpatient and day-patient: hospital treatment and surgery are in the core plan.
- Out-patient: consultations, diagnostics and therapies come through the optional out-patient module.
- Dental: provided by the optional dental module at your plan level.
- Servicing: the MyHealth app handles claims and documents, backed by a 24/7 multilingual helpline.
Two decisions to make before you look at price: which core level (and therefore which module level) you need, and which area of cover. Our guide to inpatient versus outpatient cover helps with the first; it is the difference between a hospital-only plan and one that pays for everyday appointments.
What this plan does for each benefit
Here is each of the eight benefits IPMIBenefits tracks, with what Allianz Care's Table of Benefits states. Figures for Care Base, Care Enhanced and Care Signature from Allianz Care's Table of Benefits, Care, valid from 1 November 2025, read 19 October 2026.
- Maternity: Care Enhanced and Care Signature only, after a 12-month waiting period: routine maternity US$6,750 and US$13,500 per pregnancy, complications of childbirth US$13,500 and US$20,250, complications of pregnancy covered; not on Care Base (p.11).
- Dental and optical: dental is an optional module, bought at the same level as your core plan. Optical: prescribed glasses and contact lenses, including an eye examination, on the Out-patient Plan, up to US$100, US$205 or US$405 (p.17).
- Outpatient: an optional module covering consultations, diagnostics and therapies, again at your core level. Out-patient claims are generally reimbursed through the MyHealth app.
- Mental health: in-patient and day-care psychiatry and psychotherapy on all three core plans (p.7); out-patient sessions on the Out-patient Plan, 5, 15 or 30 visits a year, with a doctor's referral for psychotherapy (p.17).
- Cancer care: oncology covered for in-patient, day-care and out-patient treatment on all three core plans, with pre-authorisation (p.10).
- Medical evacuation and repatriation: included on all three core plans, with pre-authorisation: evacuation to the nearest appropriate medical centre, or repatriation to your home country if it is inside your area of cover (p.9).
- Chronic conditions: no separate programme is described. What the documents do make clear is that cover for a condition you already have is decided by the underwriting basis you choose, which is where Allianz differs most from other insurers (see below).
- Wellness and health checks: health and wellbeing checks on the Out-patient Plan, up to US$540, US$1,080 or US$2,025 a year, plus listed cancer screening (p.16).

Underwriting: a real choice of basis
Most insurers underwrite one way. Allianz Care offers a choice, which is why it is often the answer for people with medical history. The options:
- Full Medical Underwriting (FMU)
- You declare your history (the application has 20 health questions). Allianz assesses it and tells you upfront what is covered, excluded or loaded, so you have certainty from day one.
- Moratorium
- No health questions upfront. Pre-existing conditions carry a 24-month waiting period and can become eligible once you have gone two continuous years without symptoms, treatment or advice for them.
- Continued Personal Medical Exclusions (CPME)
- If you are switching from another insurer, your existing personal exclusions carry across as they are, with no fresh underwriting of new conditions.
- Switch or continued terms
- Move onto broadly equivalent terms from a prior plan. Switch terms must be properly maintained, or Allianz may fall back to full medical underwriting.
If you have any medical history, this is the decision that changes what is covered from day one. Our pre-existing conditions guide compares the four routes; Nomi can flag which one fits, and an experienced adviser confirms the terms before you buy.
Claims and pre-authorisation
- Pre-authorisation is required for inpatient and high-cost treatment; the items that need it are flagged in the table of benefits.
- Skip required pre-authorisation and, if the claim is later found eligible, Allianz pays 80% of inpatient benefits and 50% of other benefits. It pays to authorise first.
- Emergencies do not need advance authorisation, but Allianz must be told about the admission within 48 hours.
- Invoices should be submitted within 60 days of discharge unless special arrangements are in place.
- Out-patient and dental claims are generally reimbursed through MyHealth.
Who Allianz Care suits
A strong fit if you want a global insurer behind the plan, a clean core-plus-modules structure, and above all a choice of how your history is underwritten. For a buyer with medical history, picking between FMU, moratorium and switch terms is genuinely valuable.
Worth comparing if you are price-driven at the entry level, or you would rather have a single all-in plan than a core with modules. See how it compares with Cigna Global and IMG Global, or go back to the insurer hub.
