Cigna Global vs IMG Global (GPMI), benefit by benefit

A tiered plan with fine cost control against a modular pay-for-what-you-need one. IPMIBenefits reads Cigna Global and IMG's Global Prima benefit by benefit, so you can see which insurer states what for maternity, dental, outpatient, evacuation and chronic conditions.

In short: Choose Cigna Global for a tiered plan with precise cost control, a cost share of 0% to 30% capped by an out-of-pocket maximum, cancer in the core and a nurse-led chronic condition programme. Choose IMG Global Prima if maternity or dental is the reason you are buying, both optional on every tier with stated waiting periods, or for its wider underwriting range, including Medical History Disregarded for groups.
Key takeaways
  • IMG's dental has a 6-month wait and maternity a 10-month wait; Cigna's dental module waits 3 to 18 months by treatment, and its maternity (Gold and Platinum) needs 12 months' cover.
  • Cigna's outpatient cover comes by tier or module; IMG's out-patient cover is in the core and scales by tier.
  • Cigna's evacuation and wellness benefits are optional modules; neither benefit is stated on IMG's published summary.
  • IMG works through pre-authorisation and a Guarantee of Payment; Cigna's in-network outpatient visits can often be billed directly.
  • Medical History Disregarded is available only to company or group schemes meeting a minimum number of lives, never to individuals.

Where each one wins

Cigna's edge is fine-grained control and chronic care. A cost share of 0% to 30% plus an out-of-pocket cap lets you tune the premium precisely, and the nurse-led Chronic Condition Management programme adds proactive support for conditions such as diabetes and high blood pressure. Cancer care sits in the inpatient core on every tier.

IMG's edge is modularity and underwriting breadth. Dental and routine maternity are optional on all five tiers, so you do not pay for them unless you need them, and IMG lists one of the widest sets of underwriting routes in international private medical insurance (IPMI): Full Medical Underwriting, Moratorium, CPME and, for eligible company or group schemes, Medical History Disregarded, where pre-existing conditions can be set aside entirely.

The structural difference shapes how you compare them. With Cigna you choose a tier and then modules; with IMG you choose a tier and then switch on dental or maternity with their waiting periods. Our guide to annual limits, sub-limits and waiting periods covers the timing side of that decision.

Benefit by benefit

What each insurer publishes for the eight benefits IPMIBenefits tracks: Cigna from its plans-in-detail and optional-benefits pages (read 19 October 2026), IMG from its published summary. Silence is recorded as silence.

If maternity or dental is the reason you are buying, both publish their terms and waiting periods: IMG as optional add-ons on every tier, Cigna with maternity on Gold and Platinum and dental in its Vision and Dental module. If an ongoing condition is the reason, Cigna states a programme and IMG states more underwriting routes.

Claims and direct billing

Cigna's in-network outpatient visits can often be billed directly when you present the Cigna Healthcare ID card, and most outpatient treatment needs no prior authorisation. IMG works through pre-authorisation: planned inpatient or day-patient treatment should be authorised no later than five working days before admission, after which IMG issues a Guarantee of Payment to the hospital and settles directly. Servicing runs through the MyIMG / MyALC portal with a 24-hour helpline and WhatsApp.

On cost control, Cigna's deductible, cost share and out-of-pocket cap give three levers; IMG works with deductible options. Our guide to excess, deductible and cost sharing explains the difference in a claim year.

Choose Cigna Global if…

You want to fine-tune the premium with cost share and an out-of-pocket cap, you value proactive chronic-condition support, and you want straightforward in-network direct billing with an ID card.

Choose IMG Global if…

You would rather pay only for what you need, with dental and maternity as add-ons, or you need a flexible underwriting route, especially for group cover where Medical History Disregarded can set medical history aside.

FAQ

Is Cigna or IMG cheaper?
It depends on your age, country, cover level, excess and which optional benefits you add. Because IMG is modular, leaving off dental and maternity can lower its price, but the only way to know for you is to compare both live.
Which is better for a family wanting maternity?
IMG offers maternity as an optional add-on on all tiers with a 10-month waiting period, so you pay for it only when you need it. Cigna covers routine maternity on Gold (US$7,000) and Platinum (US$14,000) once the mother has been covered for 12 months, or 24 depending on where treatment occurs. IPMIBenefits suggests adding maternity well before you expect to claim with either insurer.
Which is better for company or group cover?
IMG is often the answer for groups: its Medical History Disregarded (MHD) basis can set members' medical history aside entirely, subject to a minimum number of lives that varies by insurer. It is not available to individuals.
Which has better chronic condition support?
Cigna publishes a nurse-led Chronic Condition Management programme for conditions such as diabetes and high blood pressure. IMG does not describe a programme; with either insurer, cover for a condition you already have depends on underwriting.
